Create Issue button on Log Time dialog

We often do work, and want to track it right away, before an issue actually exists in Jira.

Therefore, propose a "create issue" button on the Log Time dialog.

    Should have the same behavior on the both My Work and Planning view

    for time log and time planning. There's not a week I plan time that I don't hit 'C' to create issue right there.
